Job Description
Community Services Assistant-Title XIX Waiver Program
Part Time and Full Time Positions

Schedule: This position will work a schedule as approved by their respective consumer or consumers’ IDT approved plan.

Supervisor: The CSM Clinical Director will be the immediate supervisor.

Duties: Perform all program plans according to Waiver Manual standards.
Attend all in-service training.
Receive, verify, sign and submit timesheets and documentation according to requirements.
Maintain a professional relationship with consumers and families.
Report any suspected consumer abuse or neglect according to agency policy and WV laws.
Provide positive community presentation by neat appearance and actions.
Maintain all required documentation according to standards and maintain all timeline mandates.
Perform activities as assigned by supervisor

Salary: To be determined based on experience, training and education.

Responsibilities: A positive working relationship must be maintained with all stakeholders in the Waiver Program.
This position must notify the immediate supervisor in the event of non-attendance at work.
The immediate supervisor will be notified immediately of any adverse incidents, injuries or property damage using the agency Adverse Incident Report form.
This position must be able to perform this job in a competent, safety conscious manner.
This position will follow all safety policies and post notices as required.
Must provide time and attendance records, activity reports and evaluations accurately on a timely basis.
Provide a current driver’s license and proof of insurance to the agency to comply with CSM Policies of utilizing personal vehicles in the performance of job duties.
Ensure the Wavier Program does not discriminate on the basis of race, color, religion, sex, national origin, handicap, age, political affiliation or ancestry.
Provide proof of U.S. citizenship and educational diplomas or general equivalency diploma.

Education: High School diploma or G.E.D. required.
